Every year, we ask families, teachers, and staff  to take the NYC School Survey. The survey gives school leaders a better understanding of what members of their school community think about their school. The information collected by the survey is designed to support a dialogue among all members of the school community about how to make the school a better place to learn.

How to Access the NYC School Survey

Step 1:

If your child attends an NYCPS or Charter School, you will need your child's nine-digit student identification number (OSIS) to begin the survey online. You can find the OSIS number on your child's report card or student ID. If you cannot locate it, contact your child's school.

If your child attends an Early Childhood Education Program, you will receive a unique access code from your child's program to take the survey. If you have not received it, contact your child's program.

Get What You Need to Start

You will need your nine-digit student identification number (OSIS) to begin the survey online.

You should have received your OSIS number from your survey proctor. You can also find your OSIS number on your report card or student ID card.

Locate Your Access Code

You will receive a unique access code from your school or program. If you have not received an access code, contact your survey coordinator.

Step 2: Enter Your Access Code

  • NYCPS and Charter School Families
    Enter a lowercase “f” followed by your child’s nine-digit OSIS number (example: f123456789).
  • Early Childhood Education Program Families
    Enter the unique access code provided by your child’s program. Do not add any letters or spaces before or after the access code.

Enter a lowercase "s" followed by your nine-digit OSIS number (Example: s123456789).

You may also access the survey by logging into your NYC School Account without requiring your child’s OSIS number or complete the survey on paper.

IMPORTANT: 

Under the updated policy, Chancellor's Regulation A-413, students are not permitted to use personal internet-enabled electronic devices during the school day.

This includes devices such as cell phones, laptops, tablets, and portable music and entertainment systems.

Student Transportation FAQ's:

Contact the Bus Company with your child's bus route number when:

  • Your child's school bus is late.
  • Your child's school bus did not arrive today.
  • You’re unsure about your child’s pick-up and/or drop-off times.

Contact OPT at (718) 392-8855 when:

  • You want to submit a complaint against a bus company.
  • You want to report a safety concern about a school bus.
  • You need help with an emergency involving a school bus.
  • You want to report an accident involving a school bus.

 

*Your child may qualify for an OMNY metro card if yellow bus service is NOT available for them. Please  speak to Ms. Sarmiento, Parent coordinator,  in the Main office.
